Why the names get muddled

In South African conversation, tick bite fever covers everything a tick might have given your dog. Medically it is at least three different diseases with different parasites, different courses and different treatments.

It matters because the treatment for one does not treat the other, and a dog can carry both at once.

Biliary, or babesiosis

Caused by a protozoal parasite that destroys red blood cells directly.

Typical picture: rapid onset over a day or two, marked lethargy, pale gums, sometimes dark urine and jaundice. The dominant problem is anaemia.

Diagnosed on a blood smear. Treated with a specific injectable parasiticide.

Ehrlichia, or canine monocytic ehrlichiosis

Caused by a bacterial organism that infects white blood cells.

Typical picture: slower and less dramatic. Fever, lethargy, weight loss, and often bleeding signs — nosebleeds, pinpoint bruising on the gums or belly, blood in the urine — because platelets drop.

It can go chronic and smoulder for months. Diagnosed on antibody testing or PCR and treated with a long course of doxycycline, usually three to four weeks.

Human tick bite fever is a different thing again

What people in South Africa get from a pepper tick, with the black eschar and the headache, is a rickettsial infection of humans. Dogs do not typically get that disease.

So when a vet says tick bite fever about your dog, ask which one they mean. The answer changes the medication and the length of the course.

What to do about it as an owner

You do not need to distinguish these yourself. What you need to recognise is the pattern: a dog with tick exposure that is lethargic, off its food, has pale gums, or is bruising or bleeding oddly.

Ask your vet which organism was found or suspected, and whether the treatment covers both. In a high-risk area many vets will treat for both while awaiting results.

Prevention is identical for all of them: year-round tick control and physical tick checks after time in grass or bush.
